phone role sink inputs and source outputs stuck in corked state

      When the fix from SPEC-660 is applied to avoid the AudioManager crash, audio streams for the phone role are left corked by an unknown entity. This prevents any audio from being heard on a dialed or incoming phone call in the phone app.
